Common LPA Registration Mistakes (and How to Avoid Them)

A practical pre-submission checklist covering execution sequence, section errors, contradictory wording, and preventable OPG requisitions.

Most rejections are process errors

In many cases, the underlying intention is fine but the execution or form logic is invalid.

High-frequency mistakes

  • Wrong signing order
  • Missing signatures or dates
  • Witness incompatibility
  • Contradictory restrictions/instructions
  • Incomplete LP1H section choices

Signature sequence matters

LPA forms have sequencing requirements. Signing out of order can invalidate registration even where all signatures exist.

Date risk control

Do not prefill speculative dates and do not "fix" mistakes by overwriting dates informally. If an execution error occurs, re-run correctly.

Address and identity consistency

Minor mismatches across pages and supporting docs can trigger requisitions. Validate names, addresses, and role descriptions line by line.

Preflight checklist

Before sending:

  1. Validate all role sections complete
  2. Validate each signature and witness section
  3. Check date order logic
  4. Check instruction consistency
  5. Check supporting details and references
